As well as supporting individuals within our shared accommodation, we offer a home care service, allowing us to provide individualised care and support to service users in their own home.
Tylersfield is an example of our home care service. Our staff support an individual living in her own flat for an agreed number of hours per day. This person is someone who had previously lived in one of our supported living services and, following successful support, was able to take the next steps in her independence and move into her own accommodation.
By providing support at home, we are able to continue the journey of those in our care, allowing them to keep developing the skills and confidence required to live independently with dedicated and individualised support. Whatever areas a person may require support in, from health and social care to general living skills or even gaining employment, we can be there to provide this support throughout their journey.
The care you need, when and where you need it
While all of the homes we provide care to may differ in appearance or function, all have a shared part in our own specialist practices and expertise. The care we provide to service users is entirely individual, covering healthcare, social care and daily living skills. Every service user will undergo individual assessment and then we will develop a bespoke care plan, which is continually reviewed to ensure we are providing the best possible support.
Whether a service user requires our assistance 24 hours a day, or just a few hours a week, we are able to provide the care that they need, when they need it. This may be in a residential care home, supported living accommodation, or a home care service. Depending on the individual goals and capabilities, some of the areas we promote independence include;
- Meal preparation and cooking
- Household management and chores
- Financial management and budgeting
- Participating in local community activities, including employment opportunities
- Continuing education, including college, vocational and non-vocational courses
